Base Pay: $139,100.00 - $157,243.00 / yearJOB SUMMARY
This position works closely with the Medical Director of HOPE, to provide health care services to individuals who are unsheltered in a variety of settings including: the mobile outreach van, homeless encampments, medical respite, hotels, homeless shelter, and streets. This position is required to perform appropriate patient assessments including the history and physical exam, ordering appropriate diagnostic tests, and implementing therapeutic treatment measures under the supervision of the HOPE Medical Director. Works as an integral part of the clinician staff by participating in Medical Staff and PA/NP meetings as well as other projects and committees of Shasta Community Health Center.
JOB DUTIES AND RESPONSIBILITIES
- Take appropriate history and physical examination of patients.
- Order pertinent diagnostics and implement appropriate therapy according to scope of practice guidelines.
- Obtain supervising physician's consultation when problems presented are outside scope of practice or when unfamiliar with a particular aspect of the patient's care according to the PA/NP Standardized Procedures.
- Communicate effectively with the patients while involved with their health care, including providing appropriate patient education while answering questions and in history taking.
- Demonstrate competent knowledge in delivering health care to patients through the peer review processes including chart review, by interactions with physician preceptors and by working collaboratively with support staff and all staff of SCHC.
- Become an integral part of the clinical staff through participation in Medical Staff meetings, PA/NP meetings, and other committees of SCHC.
- Willingly perform other duties within the scope of practice as assigned.
- Responsible for maintaining current licenses to practice medicine. Copies are to be given to the Compliance department.
- Responsible for providing proof of earned CME hours annually to the Compliance department.
- Work in mixed clinical settings, including but not limited to on the street, in homeless encampments, emergency shelters and Medical Respite programs.
- Performs other duties within the scope of practice as assigned.
KNOWLEDGE, SKILLS, AND ABILITIES
- Knowledge of current therapies and ability to involve the supervising physician when appropriate.
- Serving as the primary care physician for people experiencing homelessness or housing instability.
- Identifying systems gaps to assist in providing the department with ongoing program quality improvement.
- Independently assesses and treats acute/chronic clinical problems common in both episodic and primary care settings, as well as primary care specialty areas including but not limited to HIV, hepatitis C, sexual health, reproductive health / family planning, transgender health, behavioral health and addiction medicine, and urgent care.
- Able to provide full scope care in a variety of settings including on the street, at encampments, in temporary and permanent housing, motels, and in a clinical setting.
- Demonstrates an interest in and an ability to learn various care delivery models for patients experiencing homelessness. Must be comfortable in adapting to providing care across a variety of treatment settings.
- Supervises nursing, Medical Residents, Interns and NP students to meet objectives of clinical placements.
EDUCATION & EXPERIENCE
- Completed a nationally accredited Physician Assistant or Nurse Practitioner Program.
- 3 years of experience or post-graduate training preferred.
- California license upon hire and maintain current without any limitations on license to practice.
- Current PA certification through the National Commission on Certification of Physician
- Assistants, Inc., or NP certification through the American Nurses’ Association.
- CPR certification upon hire and maintain current.
PREFERRED QUALIFICATIONS
- 3 years of experience or post-graduate training preferred.
- ACLS certification.
- BCLS certification.
